Required
- Active LPN license
- BLS certification
- CPR certification
- Completion of an approved LPN program
- Understanding of confidentiality and nursing best practices
- Strong communication and interpersonal skills
- Ability to work with a multidisciplinary care team
- Compassion, patience, and professionalism
Preferred
- Home health experience
- Pediatric care experience
- Experience with medically fragile clients
- Familiarity with private duty nursing
- Comfort working in a home-based care setting

Responsibilities
What LPNs may support in care.
Record medical histories
Accurately document patients' medical histories.
Monitor vital signs
Take and record measurements of blood pressure, temperature, heart rate, and other required vitals.
Observe treatments
Monitor patients under treatment to identify progress and possible side effects of medications.
Maintain patient charts
Track patients' conditions, including fluid intake and output, and maintain detailed patient charts.
Assist with daily activities
Help patients with necessary daily activities such as eating and bathing.
Administer medical care
Provide injections, prescribed medications, enemas, wound cleaning and dressing, and other basic medical tasks within scope and care-plan requirements.
Offer emotional support
Provide emotional and psychological support when needed.
Communicate with families
Provide advice, comfort, and release instructions to patients' family members or friends when appropriate.
