What is the difference between HHA and FHHA?
A Home Health Aide (HHA) is an aide who assists with permitted personal care and daily activities. A Family Home Health Aide (FHHA) is an eligible family caregiver trained and employed by a participating Florida home health agency to care for a medically fragile child under 21 who qualifies for Private Duty Nursing (PDN).
The main differences are the caregiver's relationship to the child, program eligibility, training and coordination with nursing services.

What does each type of aide do?
Home Health Aide
An HHA helps a person manage daily activities at home. Depending on the care plan and permitted duties, that can include bathing, grooming, dressing, toileting, positioning, mobility and feeding assistance.
This role can support different age groups. Needing aide services does not automatically mean someone qualifies for Medicaid coverage or the FHHA program.
Family Home Health Aide
Florida's FHHA program gives eligible family caregivers a route to training and employment so they can provide authorized care for their medically fragile child.
The agency coordinates the child's services and validates the caregiver's skills. FHHA care is overseen by a non-related registered nurse.
Who qualifies for FHHA in Florida?
The child and the family caregiver must both meet the requirements. Being a parent or already providing care at home does not automatically qualify someone for paid FHHA services.
The child's requirements
- Be under age 21.
- Be medically fragile and qualify for medically necessary Private Duty Nursing.
- Have services arranged through the agency and applicable payer authorization process.
The caregiver's requirements
- Be an eligible family caregiver age 18 or older.
- Meet basic reading and writing requirements.
- Complete the applicable approved training pathway and competency validation.
- Pass required background screening and meet agency employment requirements.
Is HHA training the same as FHHA training?
No. A general HHA course or certificate should not be assumed to satisfy every FHHA requirement. Traditional HHA training requirements depend on the employment setting and applicable rules. FHHA training specifically addresses the medically fragile child's care needs.
Florida's FHHA training pathway
The statutory program requires at least 76 hours: at least 40 hours of HHA training, 20 hours of basic nursing skills tailored to the child's needs and 16 hours of supervised clinical training. The employing agency must validate competency through an RN. HIV/AIDS training and current CPR certification are also required.

How does FHHA affect Private Duty Nursing hours?
PDN is skilled nursing delivered by a licensed RN or LPN. FHHA provides authorized support within the family caregiver's training, validated competency and permitted delegation. These are different roles.
Florida Statutes § 400.4765 requires FHHA services to reduce the eligible child's PDN service hours and prohibits providing FHHA and PDN services at the same time. The agency and payer must coordinate the schedule around the child's approved needs.
Review the schedule before services begin
Ask which hours are assigned to the nurse, which are assigned to the FHHA caregiver and who to contact if the child's needs change. FHHA does not give a caregiver a nursing license or authorize them to perform every task a nurse performs.
